Nature of Services
Shivaya Healing Arts offers wellness services including yoga, sound healing, meditation, massage, reiki, and energy healing. These services are intended to support your general well-being and are complementary in nature. They are not a substitute for professional medical advice, diagnosis, or treatment. If you are experiencing a medical condition, always seek the guidance of a qualified healthcare provider before participating.

Assumption of Risk
I understand that participation in wellness services involves inherent physical and personal risks, including but not limited to physical discomfort, emotional release, muscle soreness, or adverse reactions. I acknowledge that I am voluntarily choosing to participate with full awareness of these risks.

Health Disclosure
I confirm that I have disclosed, or will disclose prior to my session, any medical conditions, injuries, surgeries, pregnancies, or other health considerations that may be relevant to my participation. I understand it is my responsibility to inform my practitioner of any changes to my health status.

Release of Liability
In consideration of being permitted to participate in services provided by Shivaya Healing Arts, I hereby release, waive, and discharge Shivaya Healing Arts, its owners, practitioners, employees, and agents from any and all claims, demands, or causes of action arising from my participation, including any injury, loss, or damage to my person or property, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Consent to Services
I voluntarily authorize Shivaya Healing Arts practitioners to perform the wellness services I have selected. I understand I may withdraw consent at any time before or during a session.

Cancellation Policy
I understand that to cancel or reschedule an appointment, I must provide at least 24 hours advance notice. Cancellations made with less than 24 hours notice may be subject to a cancellation fee at the discretion of Shivaya Healing Arts.

Acknowledgment
By proceeding with my booking, I confirm that I have read and understand this agreement, that I am 18 years of age or older (or have obtained parental/guardian consent), and that I am entering into this agreement voluntarily and of my own free will.
